Comprehensive Care

As a member, you will have a dedicated care team with an in-person promotora de salud (CHW), a virtual MD, and a virtual behavioral health specialist.

Following intake and assessment, our promotora facilitates will develop a care plan tailored to your unique medical, social, and mental health challenges.

Continual Support

A promotora will work closely with you to coordinate your care, helping with doctor’s appointments, community support services, and more.

• Referrals and coordination with specialty providers, hospitals, and community partners
